1. Sign up for the desired blended
course through the website.
2. Complete the Online course (You have
4 Months to Complete) Print the Part 1 Certificate upon completion of the
course.
3. Present the Part 1 certificate to the
skills evaluator.
4. Complete the skills evaluation, using
the paper form 1001, 1002, or 1003. Student & evaluator must sign the form.
5. Go to the appropriate link on page 2 to
complete the online submission form to Central Ohio CPR.
6. Student will receive their final
digital certification in their email.
7. Attach a copy of the following and
place it in your files for licensing review.
1. Part 1 Online course completion Form
2. Appropriate skills evaluation form
either 1001, 1002, or 1003
3. Final Certification from Central Ohio
CPR
8. Provided the student submitted their
OPIN # during the course. Central Ohio CPR will upload the training to their
respective PD Tract in OCCRRA
SUBMITTING
THE SKILLS ASSESSMENT ELECTRONICALLY TO CENTRAL OHIO CPR:
You essentially are entering the data from form 1001 or 1002, you used during
the skills assessment. This should take only a couple of minutes, and most of
that time is completing the data on page 1.
After page 1 you will notice that we have pre-checked “Yes” on
everything except the instructor statement at the end of each section. The system will not allow you to check “NO”
on “Critical Skills”
You should annotate to the left of the item checked ”NO” as to
why you checked the “no” block.
